Job Description:	Come Watch Us Grow! At Assisted, there are three separate and distinct companies which allow our patients the continuity of care they deserve. Our agency is a state licensed, JCAHO accredited home health and hospice agency with Medicare and Medi-Cal certification in most offices. Assisted has grown to employ an expansive team of over 600 nurses and paraprofessionals which consist of field, contract and administration positions. Our locations in California include Covina, North Hills, Thousand Oaks and Ventura. Assisted Home Care provides home care to patients when skilled health care in their home means independence, comfort and the possibility of a faster recovery. Services may include care provided by nurses, therapists, social workers and home health aides. As a Physical Therapist for Assisted, you will be responsible for clinical assessments, plan odf care development and case management. You must also have strong written and oral communication with interpersonal skills. Team collaboration and clinical reasoning are also necessary. This position requires a Bachelor of Science in Physical Therapy and current California PT licensure. A minimum of one year experience in a home care setting would be preferred. This is a contract position and our immediate need is in the Orange/ San Bernardino County area.
