We are actively seeking a Physical Therapist for A leading provider of home health nursing, rehabilitation and personal care services.
As a Physical Therapist you will:
Provide physical therapy services to patients in the home care setting. Approximately 25 home visits per week.
Focus primarily on assessment, quality care, monitoring of patient status, compliance and teaching of patient and caregivers and discharge planning.
Qualifications:
Must be a registered physical therapist of CT
Previous experience in the home care setting is desired
A minimum of one year PT experience
Be able transfer, maneuver and lift incapacitated clients in the home setting
